  • Name (First, Last, and Middle Initial plus any other names your account might be under)
  • CWID Number (8-digit number that can be found on the back of your ID card)
  • Birth Date (MM/DD/YYYY)
  • Contact Information (email address, non-CSC email address if you cannot access your CSC email account, and phone number)
  • Description of Your Problem (with any error messages you may have encountered)
